- Patient said they have AD(H)D symptoms
- Patient describes suffering from ADD symptoms
- Patient mentions having tried Methylphenidate (ADD drug) with great success
- Patient would like prescription for Methylphenidate to help their symptoms
- I am a psychiatrist who also does and explicitly offers ADD diagnosis in adults
